This insanity can manifest in a number of ways, as shown in the Illidan novel. After the things he's seen and done and experienced - well, any demon hunter is at least mildly insane. Now all that out of the way, my personal opinion is that I prefer the interpretation that Vandel is referring to his inner demons on a conceptual level. Now this can easily just be an oversight/unnecessary for quest text, or it can be evidence that Vandel's experience was very personal, which I elaborate a bit more on below. No where in game at all (to my knowledge) do demon hunters reference demons within them talking to them. It is also worth pointing out this tidbit exists only in the novel, which is also where the contradiction exists. Either way, it's more ambiguous than either side would let you believe. I don't have the actual quotes on hand, but I am certain someone can supply or some googling can suffice. However, since we're dealing with him literally eating a demon's essence, it's less clear. If this were a different story, it would be much more obvious that Vandel is referring to his conceptual inner demons, i.e. The wording gets complicated, because we're dealing with literal demons.

Later in the story, however, Vandel comes to (believe or understand, depending on your viewpoint) that the voice is not the felhound, but instead a manifestation of inner demons. In Illidan, Vandel initially interprets the voice inside his head as the fel hound's spirit that he killed and consumed. I've seen arguments on either side, and while I personally lean towards "artistic license" in the wording, I can understand where people prefer a literal interpretation. I think this is one of those things that, unless Blizzard does a hard ruling on it, can be interpreted either way. So, Demon Hunters having a literal demon within them that talks to them is debatable.
